In a significant medical achievement, a team of surgeons at Shree Krishna Hospital in Karamsad has successfully performed a complex operation on a 49-year-old man suffering from a rare and advanced form of kidney cancer. The challenging case involved a tumour that had spread from the kidney into a major blood vessel leading directly to the heart.

A Critical Case from Anand District

The patient, a resident of Anand district, was initially admitted to a hospital in Nadiad in a critical state, complaining of persistent and severe abdominal pain. He was subsequently referred to Shree Krishna Hospital for advanced care. After a thorough and detailed evaluation, the medical team diagnosed him with renal cell carcinoma (RCC) with venous invasion. This specific condition meant the cancerous growth had extended from the kidney into the inferior vena cava (IVC), the large vein responsible for carrying blood from the lower body back to the heart.

The Surgical Challenge and Multidisciplinary Strategy

Such cases where a tumour creates a thrombus (clot) inside the IVC are uncommon and present formidable surgical risks. The complexity was heightened in this instance as the tumour thrombus was located perilously close to the liver, increasing the danger of damaging vital surrounding structures during removal.

Explaining the diagnostic intricacies, urologist Dr Kaushal Patel emphasised that distinguishing a tumour thrombus from a regular blood clot on imaging scans requires meticulous analysis. "Accurate diagnosis using advanced multimodal imaging is crucial," Dr Patel stated, noting that even a minor error could drastically alter the treatment pathway. This was not a routine clot but a direct extension of the cancerous tumour.

Given the high-stakes nature of the procedure, the hospital adopted a multidisciplinary approach. A specialised team was assembled, bringing together expertise from urology, vascular surgery, oncology, and anaesthesiology. The surgery was led by urologists Dr Kaushal Patel and Dr Priyesh Patel, along with vascular surgeon Dr Jayesh Patel. They were supported by oncologist Dr G C Raghunandan and anaesthesiologist Dr Ushma Parikh.

A Successful Outcome and Patient Recovery

The anaesthesia team administered special central line-based anaesthesia to manage pain and maintain the patient's blood stability throughout the lengthy and delicate operation. The surgical team then meticulously removed the affected kidney along with the cancerous thrombus from the IVC, taking extreme care to preserve all adjacent vital organs and structures.

The procedure was completed without any complications. "The surgery was completed without complications. The patient recovered well after the operation, remained stable during post-operative observation and was discharged in satisfactory condition," confirmed Dr G C Raghunandan. This successful intervention highlights the capability of tertiary care centres in Gujarat to manage highly complex oncological cases through collaborative expertise.
