Why Are Doctors Struggling with Empathy?

Many doctors openly admit that their empathy declines over time, and patient ratings of healthcare professionals' empathy show significant variation. Multiple inquiries into healthcare tragedies in the UK have explicitly identified lack of empathy from healthcare professionals as contributing to avoidable harm.
The real issue, however, lies in the system we've created. Doctors spend approximately one-third of their time on paperwork and electronic health records rather than direct patient care. They cannot sit in meaningful silence with a dying patient when words fail, share moments of dark humor that break tension, or exercise the moral judgment required when clinical guidelines conflict with a patient's values.
These human elements aren't minor additions to healthcare – they're often what makes care truly effective, healing possible, and medicine humane.
The Path Forward: Three Essential Changes
The current trajectory points toward a concerning future where AI provides the empathy while exhausted humans handle technical work – exactly the opposite of what would serve patients best. Addressing this requires three fundamental changes in our approach to healthcare.
First, we must train doctors to excel consistently at empathic communication. This cannot remain a brief module in medical school but needs to become central to healthcare education. Since AI already matches humans in many technical skills, this should free doctors to focus on genuine human connection.
Second, healthcare systems require redesign to protect the conditions necessary for empathy. This means dramatically reducing administrative burden through better technology, ensuring adequate consultation time, and addressing burnout through systemic change rather than resilience training alone.
Third, we need rigorous measurement of both benefits and harms of AI in healthcare interactions. Future research must focus on actual patient outcomes, missed diagnoses, inappropriate advice, and long-term effects on therapeutic relationships – not just whether responses sound empathetic to researchers.
The empathy crisis in healthcare isn't caused by insufficient technology but by systems that prevent humans from being human. AI appearing more empathetic than doctors represents a symptom, not the disease itself.
