Application Process

Interested applicants can visit the Gandhinagar Head Office or sub-offices in Surat and Valsad during office hours to request a hard copy of the application form. The staff responsible for issuing and collecting forms will provide assistance. After filling in all required fields, applicants must attach copies of mandatory documents, which may need self-attestation. The completed form, along with documents, must be submitted to the sub-offices in Surat or Valsad. Upon submission, applicants should request a receipt or acknowledgment containing the date, time, and a unique identification number if applicable.
