Kerala Government Approves Unified Surgical Safety Protocol for All State Hospitals

In a significant move to bolster patient safety and standardize surgical care, the Kerala state government has given its approval for a comprehensive, unified surgical protocol to be implemented across all government healthcare institutions. This decision comes in response to a series of reported surgical errors in government hospitals, aiming to address these issues proactively.

Protocol Development and Global Alignment

Prepared by the directorate of medical education, the protocol is meticulously designed to align with global safety guidelines issued by the World Health Organization (WHO), while also incorporating tailored measures specific to Kerala's healthcare system. The draft was recently submitted to the government, which has now directed authorities to expedite its rollout to ensure swift implementation.

Officials emphasized that the protocol is crafted to minimize surgical errors, improve coordination among medical teams, and ensure consistent adherence to safety procedures from preoperative stages through to post-surgical care. This initiative is expected to significantly enhance the quality and accountability of surgical procedures in the state.

Key Components of the New Guidelines

Under the new guidelines, ward doctors and nurses are required to complete a series of checks either on the night before surgery or early on the day of the procedure. These checks include:

  • Verifying patient identity using multiple identifiers to prevent misidentification.
  • Cross-checking medical records to ensure accuracy and completeness.
  • Ensuring informed consent is obtained after proper counselling, emphasizing patient autonomy and understanding.

Additionally, patients will be given colour-coded wristbands that indicate whether the procedure is elective or an emergency, along with key medical details. To prevent wrong-site surgeries, mandatory marking of the surgical site in the preoperative ward has been introduced. A detailed preoperative checklist must be completed and verified before the patient is moved to the operating theatre.

Structured Operating Room System

The protocol introduces a structured operating room system based on internationally recognized practices, including a three-stage checklist: sign-in, time-out, and sign-out, applicable to all types of anaesthesia.

  1. Sign-in Phase: Conducted before anaesthesia, this phase requires confirmation of patient identity, surgical site, and procedure details. Strict infection control practices and medication safety checks are also mandated during this stage.
  2. Time-out Phase: Carried out before incision, all members of the surgical team must confirm patient details, procedure specifics, allergies, and the availability of required equipment. In cases involving multiple procedures, additional time-outs must be conducted before each stage to maintain vigilance.
  3. Sign-out Phase: Conducted after surgery, this phase focuses on accurate documentation, patient transfer decisions, and verification of all surgical instruments and materials. Strict counting and cross-checking of instruments and disposables are mandated to prevent retention errors, ensuring nothing is left behind.

Flexibility and Implementation

While establishing a uniform framework, the protocol allows departments to adapt checklists to their specific requirements. However, any modifications must be formally submitted for inclusion in institutional protocols to maintain consistency and oversight. Health officials have stated that this initiative is expected to further strengthen Kerala's public healthcare system by improving safety, accountability, and quality in surgical procedures.

Departments have been instructed to take urgent steps to ensure implementation across all medical facilities, with a focus on training and compliance to achieve the desired outcomes. This proactive approach underscores the government's commitment to enhancing healthcare standards and protecting patient well-being in the state.
